Honorable discharge on medical grounds. Benefits as a Dominion War veteran. Still, the boot.
Of course the design is on purpose, nothing Garak does is accidental. To remind everyone how much he did as a doctor yet how Starfleet just discarded him and how Cardassia is lucky to have him instead.
